
Primary Care Registered Nurse, RN

This is a fully remote position, open to applicants in New Mexico.
• Identify patients with high risk and high acuity who would benefit from coordinated care.
• Collaborate with providers and team members to create interdisciplinary care plans.
• Provide orientation and education to patients and family/support members regarding the care coordination process.
• Coordinate care information and requirements with other healthcare providers.
• Address issues impacting care progression and promote peer support and education.
• Assess adherence to and effectiveness of care plans, making modifications as necessary.
• Evaluate the effectiveness of interventions.
• Assist with appointment scheduling and the completion of tests or procedures.
• Remove or reduce obstacles to adherence to care.
• Deliver clinical care in accordance with SCC policy and the New Mexico Nurse Practice Act when required.
• Refer patients to additional providers or resources for specialized needs.
• Accurately and promptly document in the EMR.
• Gather and analyze data that supports clinical best practices and SCC Quality programs.
• Assist with research studies related to care protocols, care planning, and interventions.
• Attend staff meetings, conferences, and seminars.
• Carry out other related duties as assigned.
• A minimum of 1 year of RN experience in an ambulatory or primary care setting is preferred.
• LPN or Medical Assisting experience may be accepted in place of the RN experience requirement.
• An associate degree in nursing is required.
• A Bachelor of Nursing degree is preferred.
• A valid Registered Nurse License in New Mexico or another compact state is necessary.
• Candidates must reside in the Albuquerque or Santa Fe, New Mexico area.
• Ability to document in the EMR in a timely and appropriate manner.
• Capability to provide clinical care in line with SCC policy and the New Mexico Nurse Practice Act.
