Physician Owner – President, Affiliated Medical Group

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Act as the sole or primary shareholder, director, and President of one or more professional corporations specific to each state.

• Approve clinical protocols, quality standards, and policies regarding the scope of practice for NP-delivered care.

• Implement and oversee collaborative practice or supervision agreements with nurse practitioners as mandated by state regulations.

• Supervise credentialing, peer review, and clinical quality committees.

• Serve as or appoint a Medical Director for relevant programs.

• Authorize clinical policies, standing orders, and care pathways.

• Ensure Medicare enrollment for the professional corporation and manage the reassignment of billing rights.

• Collaborate with the MSO to ensure compliant operations while maintaining exclusive authority over clinical decisions.

• Engage in periodic governance meetings, audits, and regulatory inquiries as necessary.

⛳️ Requirements

• MD or DO with an active and unrestricted medical license.

• Willingness to acquire licensure in additional targeted states.

• Active and unrestricted DEA registration.

• Board certification in Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine & Rehabilitation is preferred.

• Current Medicare enrollment or eligibility without any revocations, opt-outs, or payment suspensions.

• Clean compliance background: no exclusions from OIG/SAM, no disciplinary actions from state boards, and no significant malpractice history.

• Experience in supervising nurse practitioners and/or serving as Medical Director in post-acute, SNF, home health, telehealth, or value-based care environments.

• Familiarity with friendly-PC arrangements, stock transfer restrictions, succession agreements, and MSO management frameworks.

• Previous ownership of a telehealth, RPM, or digital health company is a plus.

• Multi-state licensure or participation in IMLC is advantageous.

• Background as a SNFist or post-acute medical director, along with knowledge of PDPM, RPM/CCM billing codes, and CMS Conditions of Participation is a plus.

• Experience with quality programs, UM/peer review, and payer credentialing is a plus.
