This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create personalized treatment plans.
• Provide physical therapy services as outlined in the physician’s care plan and in compliance with California’s Physical Therapy Practice Act.
• Inform patients and their families or caregivers about therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the care team, communicate changes in patient status, and engage in case discussions.
• Oversee Physical Therapy Assistants and, when PT is the only skilled service, manage Home Health Aide care on a bi-weekly basis.
• Handle documentation including clinical progress reports and discharge planning.
• Recognize the need for additional home health services and facilitate referrals as necessary.
• Execute other tasks as needed for patient care.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Current California Physical Therapy license.
• At least 2 years of experience in physical therapy; experience in home health is an advantage.
• Excellent communication skills with the ability to engage effectively with patients and their families.
• Up-to-date CPR certification with AHA.
• Valid driver’s license and a reliable, insured vehicle.
