This is a fully remote position, open to applicants in Tennessee.
• Serve as mental health support for referred patients and individuals seeking mental health services.
• Communicate with patients regarding behavioral health inquiries through the Diana Health platform.
• Conduct both in-office and telehealth mental health consultations via warm hand-offs from providers and/or at the request of patients.
• Deliver ongoing mental health treatment and make referrals as necessary.
• Ensure timely completion of clinical documentation in the EHR system.
• Work collaboratively with CNM/OB-Gyn to develop care plans, address escalation areas, and assess medication needs.
• Partner with the Health Coach to enhance patient wellness.
• Collaborate with the Care Navigator to offer additional support to patients.
• Provide training for staff on behavioral health practices and workflows.
• Facilitate behavioral health expert discussions and support groups.
• Connect patients and providers with external referrals and resources.
• Licensed Clinical Social Worker (LCSW) or Marriage Family Therapist (MFT) with valid licensure in Tennessee.
• Minimum of 2 years of experience delivering mental health support to a comparable client demographic.
• Outstanding communication, interpersonal, and organizational abilities.
• Proficient computer skills and experience with EMRs.
• Expertise in postpartum or trauma care or proficiency in EMDR or IFS therapy, among others.
• Utilize a therapeutic approach distinct from CBT or DBT.
• Must possess a Tennessee license or be eligible to obtain one upon acceptance of the offer.
