
Physical Therapist Assistant, Colorado License Required

This is a fully remote position, open to applicants in Colorado.
• Implement personalized physical therapy treatment plans as established and overseen by the supervising Physical Therapist.
• Provide therapeutic interventions including therapeutic exercises, gait training, balance activities, transfer training, and modality application.
• Monitor and document patient progress, functional status, and treatment responses in accordance with state, federal, and facility standards.
• Regularly communicate with the supervising PT, interdisciplinary team, and nursing staff regarding resident goals and progress.
• Educate patients, families, and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.
• Participate in care planning meetings, utilization reviews, and MDS documentation as applicable.
• Maintain timely and accurate clinical documentation in line with Medicare/Medicaid and facility requirements.
• Uphold resident rights, dignity, and confidentiality in all interactions.
• Assist in keeping a clean, organized, and safe therapy gym and treatment environment.
• Associate's degree from an accredited PTA program.
• Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to the start date).
• Current CPR/BLS certification.
• Strong interpersonal and communication skills with a patient-centered approach.
• 1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation setting (preferred).
• Familiarity with MDS/PDPM documentation frameworks (preferred).
• Experience with geriatric populations and fall prevention programs (preferred).
• Knowledge of EMR/documentation systems utilized in SNF environments (preferred).
