This is a fully remote position, open to applicants in North Carolina.
• Engage with members who qualify for the TCL special needs population to educate them about permanent supportive housing, available supports, services, and community resources.
• Collaborate with Diversion staff, Transition Coordinators, and the Next Step Rehousing Team to assist in transition planning and facilitate movement through the TCL process for individuals who wish to secure permanent supportive housing in the community.
• Work alongside the Trillium Integrated Care Nurse, OT, COTA, and Clinician to identify complex care requirements and the necessary clinical service connections.
• Promote community integration, connect members to services, and offer education and support for those who opt not to transition to permanent supportive housing within the community.
• Apply person-centered planning techniques to collect information and understand the members and their legally responsible parties to effectively address each individual's specific needs.
• Provide education and support to members and their guardians regarding their rights and responsibilities.
• A High School Diploma or GED is required.
• Must be an individual with at least 1 year of lived experience related to a behavioral health condition and/or substance use.
• Possession of a North Carolina Peer Support Specialist Certification is mandatory.
• A valid driver’s license is required.
• Residency within Trillium’s Northern or North Central Region is essential, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington, and Wilson.
• Ability to travel within the designated catchment area as needed.
