This is a fully remote position, open to applicants in California.
• Perform high-volume outbound calls to potential CalAIM members identified via health plan referrals, community referrals, and internal lead lists.
• Articulate WEA services, Enhanced Care Management (ECM), and Community Supports programs to eligible candidates.
• Establish rapport and engage with individuals who may be reluctant or challenging to contact.
• Assess potential participants for program eligibility and interest.
• Arrange enrollment appointments and facilitate warm transitions to Care Managers and other clinical team members.
• Accurately document outreach activities, patient interactions, and enrollment results in designated systems.
• Adhere to productivity benchmarks related to outreach volume, contact rates, and enrollment objectives.
• Follow established workflows, scripts, and compliance guidelines concerning patient communication.
• Collaborate with supervisors and program staff to enhance outreach effectiveness and patient engagement.
• Uphold confidentiality and comply with HIPAA and relevant privacy regulations.
• Participate in team meetings, training sessions, and initiatives for quality improvement.
• Achieve daily outreach targets, including outbound call volume, successful contacts, and enrollment conversion goals.
• Accurately track outreach results and disposition codes.
• Complete follow-up activities within designated turnaround times.
• Perform additional duties as assigned.
• High school diploma or equivalent.
• At least one (1) year of patient-centered care experience in patient services, healthcare outreach, call center operations, care coordination, or a related field.
• Exceptional verbal communication and active listening abilities.
• Strong organizational and documentation capabilities.
• Ability to work independently in a remote setting.
• Proficiency in Microsoft Office and web-based applications.
• Capacity to handle a high volume of outbound calls while maintaining professionalism and empathy.
• Preference for candidates based in California.
• Availability to work in the PST timezone.
• Must have a private, HIPAA-compliant workspace with reliable internet access.
• Ability to communicate effectively via telephone and video conferencing platforms.
• Preferred experience working with Medicaid/Medi-Cal populations.
• Preferred experience with CalAIM, Enhanced Care Management (ECM), Community Supports, or population health programs.
• Bilingual proficiency in English and Spanish is strongly preferred.
• Preferred experience using electronic health records (EHRs), CRM systems, or care management platforms.
• Knowledge of HIPAA, CalAIM, and health plan contractual documentation and privacy compliance.
