Nurse Practitioner – Virtual Health Assessment Team

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Conduct preventative care screenings, perform medication reconciliation, diagnose medical conditions, and offer health coaching, referrals, and treatment when clinically appropriate.

• Reach out to members who are more chronically ill based on historical and anticipated health information.

• Engage with members who are less likely to visit their PCP.

• Identify opportunities in the member's medical journey that can be enhanced by improving their access to care with a licensed provider.

• Participate in cross-training for urgent care and/or virtual primary care service lines.

• Engage with members through virtual interactions, including virtual assessments, diagnoses, and treatments.

• Ensure compliance with all relevant laws and regulations.

• Perform other assigned duties.

⛳️ Requirements

• PA or DNP, FNP, ENP from an accredited program.

• Board Certification (AANP, ANCC, or NCCPA).

• Compact RN (for NP only).

• Experience in Clinical Documentation Improvement and/or managing HEDIS quality measures.

• For PA: Licensed in Georgia and one of the following states: Florida, Texas, or Ohio.

• For NP: Licensed in Florida plus one of the following states: Texas or Ohio.

• Capability to obtain licensure in additional states.

• Over 3 years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
