Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct health evaluations in the comfort of individuals' homes.

• Facilitate virtual appointments as necessary.

• Infuse compassion into every appointment.

• Collaborate with Signify Health as a valued clinician partner.

⛳️ Requirements

• Must possess an active, unrestricted license(s) in the relevant coverage area(s).

• Completion of 2080 hours of post-graduate collaborative practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in an integrated clinical care environment (this is mandatory, along with submitting the appropriate attestation to the state of Minnesota).

• Board certification is required.

• Multi-state licenses are preferred.
