Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Maine.

📋 Description

• Conduct In-Home Health Evaluations at patients' residences.

• Interact with individuals on a personal level and address health inquiries.

• Assess medications and review medical history.

• Execute physical evaluations.

• Carry out additional diagnostic tests as requested by the health plan, which may include diabetic eye exams and spirometry.

• Utilize an iPad and follow clinical workflows for each assessment.

• Travel locally and within authorized states as required.

• Conduct virtual consultations as necessary.

• Offer Signify Health services, which encompass Diagnostic & Preventive Services like spirometry, DEE, and bone density screenings.

• Assist in bridging care gaps and connecting individuals to the right healthcare resources.

⛳️ Requirements

• An active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Must be able to cover the primary location of Penobscot, ME.

• Qualification as a Nurse Practitioner or Physician Assistant is required.

• Must be willing to travel locally and within licensed states as necessary.
