Nurse Case Manager I – 100% Virtual

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Managing care within the parameters of licensure for individuals with intricate and chronic healthcare requirements

• Evaluating, developing, executing, coordinating, monitoring, and assessing care plans aimed at enhancing member health across the care continuum

• Conducting evaluations to determine specific individual needs

• Executing the care plan by securing authorizations/referrals as necessary

• Coordinating both internal and external resources to address identified needs

• Monitoring and assessing the effectiveness of the care management strategy

• Collaborating with Medical Directors and Physician Advisors regarding treatment plans

⛳️ Requirements

• Bachelor's degree in a health-related discipline and at least 3 years of clinical experience

• Valid, unrestricted RN license in the relevant state(s) is mandatory

• Multi-state licensure is required for services provided in various states

• Case Manager certification is preferred
