Nurse Care Associate

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Perform telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care initiatives.

• Assess biometric alerts and pinpoint out-of-range readings; escalate clinical issues as per established protocols.

• Deliver patient education and coaching to foster adherence to care plans and lifestyle suggestions.

• Precisely document patient interactions in EMR/RPM systems in accordance with clinical and regulatory guidelines.

• Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to ensure effective patient care coordination.

• Assist in patient engagement efforts aimed at enhancing monitoring compliance and clinical results.

⛳️ Requirements

• Active LPN/LVN license in good standing.

• 5+ years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Excellent telephonic communication, patient education, and motivational interviewing abilities.

• Familiarity with EMR/EHR systems and a willingness to operate within a technology-driven care environment.

• Capability to work independently within a structured remote clinical workflow.
