This is a fully remote position, open to applicants in United States.

• Oversee a portfolio of clients with low to moderate healthcare needs throughout their treatment journeys.

• Provide education, advocacy, care coordination, and continuous support to clients.

• Establish trustworthy relationships with clients, acting as a dependable guide in their healthcare experience.

• Assist clients in accessing suitable providers, facilities, and services.

• Collect and synthesize clinical data, preparing summaries for internal teams.

• Facilitate client transitions and ensure continuity across clinical and operational teams.

• Research and recommend providers and facilities tailored to client requirements.

• Organize appointments, manage records, referrals, and other logistical aspects of care.

• Engage in communication with clients, caregivers, providers, and internal stakeholders.

• Collaborate across functions to ensure a cohesive client experience.

• Valid Registered Nurse license in the state of residence.

• Minimum of five years of experience in healthcare.

• Background in case management, care coordination, patient navigation, or a comparable clinical setting.

• Availability to work consistently from Monday to Friday, 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time.

• Experience in a remote working environment.

• Display confidence, empathy, and strong clinical judgment.

• Excellent written and verbal communication abilities.

• Capability to simplify complex clinical information for both clinical and non-clinical audiences.

• Self-motivated with a collaborative mindset.
