This is a fully remote position, open to applicants in North Carolina.
• Engage in ongoing telephonic outreach to identified and referred members, aiming to connect them with care management services.
• Manage and receive referrals from data reports, clinical care management team members, primary care providers (PCP), or other service providers to ensure efficient screening and linkage of members with care management.
• Complete and record member screening tools, directing clinical needs to the most suitable clinical care management team member for assessment and follow-up; schedule members for face-to-face or telephonic follow-up encounters for care management.
• Achieve daily and monthly targets for program productivity and member engagement metrics.
• Fulfill all assigned tasks within the designated timelines; promptly inform the supervisor of any challenges related to receiving or sending referrals, making timely care management assignments, or executing any other assigned responsibilities.
• Adhere to departmental guidelines, company policies, and HIPAA regulations.
• Perform additional tasks that contribute to maintaining an organized and efficient operation.
• Participate in departmental and corporate meetings.
• Comprehend and support the goals, objectives, and standards of CCNC.
• High School diploma or GED.
• Previous experience in a telephonic call center is preferred.
• At least two years of prior work experience, ideally in a healthcare environment.
• Familiarity with healthcare and medical terminology is highly preferred.
• Bilingual candidates are preferred.
• Experience in managed care is a plus.
