This is a fully remote position, open to applicants in United States.
• Evaluate, strategize, implement, coordinate, monitor, and assess case management services and options to meet members’ health requirements.
• Collaborate with various departments, healthcare providers, and community organizations to ensure effective and efficient patient care.
• Act as a liaison to enhance the quality of care, cost efficiency, accessibility, and suitable service levels.
• Operate within the scope of licensure and adhere to all relevant policies, procedures, regulations, URAC standards, and state laws.
• Stay updated on social work licensing criteria and continuing education mandates.
• Provide support and resources for non-clinical personnel.
• Required to travel to locations or conduct home visits as necessary.
• Actively manage referrals from various sources, identify candidates for case management, explain available services, secure voluntary consent, and maintain an efficient patient caseload.
• Carry out additional responsibilities as assigned.
• Master’s degree in Social Work.
• Active Licensed Master Social Worker (LMSW) license in good standing in the state(s) where job duties are performed.
• Case Management Certification is mandatory; if not obtained prior to employment, successful completion of testing is required within three years.
• Valid driver’s license.
• Preference for Arkansas Licensed Clinical Social Worker (LCSW).
• At least two years of clinical practice experience providing direct care to patients in a hospital, facility, or community setting.
• Minimum of two years’ experience in case management, home health, critical care, social work, discharge planning, or utilization review.
• Proficient in Microsoft Office applications, including Word, Excel, and Outlook.
• Able to navigate multiple systems and perform keyboard functions.
• Capable of making informed judgments and decisions based on established guidelines and facts.
• Skilled in building collaborative relationships.
• Strong attention to detail.
• Competence in interpreting complex documentation.
• Ability to work independently with minimal supervision.
• Commitment to maintaining the security and confidentiality of records and information.
• Adherence to segregation-of-duties protocols.
• Willingness to travel for work and meet occasional lifting and carrying requirements of up to 40 lbs.
