This is a fully remote position, open to applicants in United States.

📋 Description

• Lead the multi-specialty polychronic care model in patients' homes.

• Collaborate to provide specialty care in the home environment.

• Emphasize patient experience, population health outcomes, provider satisfaction, and cost reduction.

• Supervise daily clinical and business operations.

• Review and authorize care plans and treatment strategies.

• Engage with community-based physicians and healthcare facilities.

• Provide direct oversight of clinical and operational team members.

• Manage patient care and treatment processes.


⛳️ Requirements

• Must be willing and able to secure hospital privileges at designated facilities.

• Proven experience in applying evidence-based clinical criteria.

• Background in renal care and geriatrics is essential.

• Strong management and communication abilities are required.

• An active, unrestricted medical license is mandatory in each state within the market.

• Experience with high-need Medicare Advantage and managed Medicaid populations is necessary.

• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment is important.

• Current, unrestricted state medical license without any sanctions from Medicaid or Medicare is needed.

• Willingness to obtain licensure in multiple states is essential.

• An MD (Medical Doctor) or DO degree from an accredited medical institution is required.

• Board Certified (BC) or Board Eligible (BE) in an ACGME-accredited specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
