LPN Inbound Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop tailored care plans for individual patients and deliver care management and coordination services utilizing Ennoble Care’s electronic medical record system.

• Maintain regular communication with patients to offer care coordination and support while ensuring adherence to care management programs.

• Accurately record all communications with clients.

• Liaise with the Primary Care Clinician or designated representative regarding urgent needs and critical incidents, ensuring comprehensive and precise care plans are maintained.

• Keep the Team Supervisor updated on issues relevant to the care plan process.

• Collaborate with various organizational departments to address the needs of patients, their families, and partner facilities.

• Utilize technology, including secure email, telecommunication systems, and electronic medical records.

• Follow documentation protocols and best practices for daily work logs, issue escalation, and internal communication.

• Deliver outstanding customer service.

• Support fellow department team members in a productive and constructive manner.

• Operate computers, telephones, copiers, and other necessary office equipment.

⛳️ Requirements

• Must hold a Licensed Practical Nurse (LPN) certification.

• Comfortable engaging in phone conversations for extended periods throughout the day.

• Demonstrate compassion and empathy towards patients, coupled with exceptional customer service skills.

• Capable of taking precise notes and documenting tasks promptly.

• Ability to manage multiple patients and workstreams while maintaining organization and efficiency.

• Thrive in a fast-paced work environment.

• Availability to work Saturday and Sunday from 8:00am–4:30pm EST and Monday, Tuesday, and Wednesday from 9:30am–6:00pm EST.

• Proficient in using computers, Outlook, and other Microsoft Office applications.

• Familiarity with basic healthcare terminology, conditions, roles, and care principles is advantageous.

• Must successfully pass a drug screening and background check.

• Exhibit a positive attitude, adaptability, teamwork, and a readiness to continually learn new skills.
