This is a fully remote position, open to applicants in United States.
• Evaluating patient conditions: Perform monthly evaluations of patients with chronic illnesses to detect shifts in their status, needs, and concerns.
• Creating care plans: Collaborate with the patient, their family, and the healthcare team to design and execute tailored care plans that cater to the patient’s specific needs and aspirations.
• Care coordination: Work alongside other healthcare professionals, including physicians, specialists, and fellow nurses, to provide comprehensive and continuous care.
• Patient education: Inform patients and their families about self-management practices, medication management, and health promotion techniques to enhance health outcomes.
• Monitor vital signs for patients enrolled in remote patient monitoring programs.
• Accurately document relevant information in the medical record as necessary.
• Gather adherence percentage data for the patient’s quarterly consultation with their provider.
• Collaborate with a registered nurse to revise the patient care plan.
• Assist in establishing new patient charts.
• Make patient calls as required.
• Precisely and comprehensively record all findings, assessments, and discussions in the electronic health record (EHR) system.
• Maintain clear and professional communication with patients during virtual visits and appointment scheduling.
• Serve as a liaison between patients and healthcare providers, relaying pertinent information to ensure continuity of care.
• Engage with patients in a compassionate and empathetic manner to foster trust and rapport.
• Demonstrate effective task prioritization in a dynamic healthcare setting.
• Efficiently manage patient schedules, follow-ups, and care plans.
• Valid LPN/LVN multi-state license.
• Proficient in using remote healthcare platforms and virtual communication tools.
• Strong communication and interpersonal skills, with the ability to build rapport with patients remotely.
• Excellent organizational and time-management skills to effectively manage virtual visits and appointment schedules.
• Knowledge of medical terminology, fundamental health assessments, and health education.
• Familiarity with electronic health record (EHR) systems and documentation practices.
• Availability to work the designated schedule and any reasonable modifications communicated by the supervisor in line with business requirements.
• Adhere to instructions and complete assigned tasks in accordance with company policies, procedures, and performance expectations.
