This is a fully remote position, open to applicants in United States.
• Log into the Claris Companion platform every day to oversee RPM data and clinical alerts, triaging out-of-range readings and promptly contacting patients requiring assistance.
• Assist patients in maintaining their engagement with monitoring to fulfill program requirements and, more importantly, to gain from the care provided.
• Provide CCM and APCM services: create and uphold patient-centered, comprehensive care plans; conduct regular check-ins; perform medication reconciliation; and facilitate care transitions along with referrals to community or social services.
• Establish trustworthy, motivational relationships with patients through open-ended, patient-focused conversations, empathetically addressing sensitive topics and social determinants of health (SDOH).
• Accurately document every interaction and log service start and end times to comply with each program’s documentation and billing standards; input billing and diagnosis codes for the services rendered.
• Keep audit-ready records while consistently adhering to HIPAA and program compliance regulations.
• Collaborate closely with the supervising practitioner and clinic team, timely escalating patients who require provider attention.
• Support continuity of care and meet the programs’ patient-access expectations as scheduled.
• Arrange follow-up contacts when not all required services can be completed during a single session.
• Seek clarification when needed, ensuring confirmation instead of assumption.
• Act as your patients’ strongest advocate — rejoice in their small victories and milestones!
• Active, unencumbered RN or LPN license with multistate privilege through the Nurse Licensure Compact (NLC).
• Bilingual (Spanish).
• Willingness and ability to acquire additional individual state licenses as required by the patient population — nurses must be licensed in the state where the patient is located. A California and/or District of Columbia license is a significant advantage.
• At least 3 years of LPN/RN experience; direct experience in providing remote care (RPM, CCM, and/or APCM) is essential.
• Proficient in EHRs and patient-monitoring/care-management software, demonstrating strong computer and typing skills. Familiarity with eClinical Works, Athena, and Epic EHRs is a benefit.
• Understanding of Medicare care-management programs along with their documentation and billing requirements — or the ability to quickly learn them.
• Comfortable working independently with general supervision from the billing practitioner.
• Outstanding listening skills and empathy, capable of asking patient-centered, open-ended questions and understanding issues from the patient's viewpoint.
• Highly organized and detail-oriented, with effective time management across a patient panel.
• Professional, warm, and personable in every patient interaction.
• Care or case management certification (e.g., CCM — Certified Case Manager) is advantageous.
• Disease-specific certification (e.g., CDCES — Certified Diabetes Care and Education Specialist) is beneficial.
• Experience with underserved, community health, or FQHC patient populations and addressing SDOH is a plus.
• Training or experience in motivational interviewing is a plus.
• Full-time 1099 position for independent contractors starting at $40.00 USD/hour, allowing you to set your own hours while coordinating your availability with your patients’ needs.
• Opportunity to transition to a Full-time Permanent Employee position in the future.
• Remote work available anywhere in the U.S., contingent upon holding the necessary state license(s).
