Licensed Physical Therapy Assistant, Care Navigator

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver remote monitoring services to assist patients as they advance through their home-exercise therapy programs.

• Guide and support users in therapeutic exercises, neuromuscular re-education, and functional training via our technology platform.

• Offer continuous feedback to the evaluating clinician regarding patient progress outside of the clinical setting.

• Maintain essential records and notes while conducting remote monitoring.

• Contribute to enhancing processes and the functionality of the platform.

• Organize and oversee a caseload while ensuring efficiency.

⛳️ Requirements

• Over 2 years of experience in musculoskeletal health is advantageous.

• Graduated from an accredited physical therapy program.

• Possess an active PTA license in a compact state.

• Tech-savvy, able to thrive in a multi-platform environment, and adept at transitioning between systems (EHR, messaging, scheduling).

• Previous experience in behavioral modification; Health Coach Certification is a plus.

• Exceptional communication, interpersonal, and presentation skills, with the ability to gather and convey meaningful data to inform decisions.

• Strong relationship-building and listening abilities.

• A demonstrated patient focus; a sincere passion for the user and their experience.

• Capacity to juggle multiple tasks and manage various types of users.

• Ability to work independently with minimal supervision.

• Flexibility and adaptability to work cross-functionally in a fast-paced environment.

• Capability to influence change and bring enthusiasm and excitement to the workplace.

• Independent and proactive work style; quick and flexible thinking with an open, inquisitive mindset.
