Hospice Triage RN, PT

This is a fully remote position, open to applicants in Utah.

📋 Description

• Provide telephone triage for patients and families in hospice care.

• Evaluate patient conditions and identify appropriate next steps.

• Collaborate with on-call teams to facilitate care and resource management.

• Document all communications and interventions with precision.

• Maintain a calm and professional demeanor while managing urgent calls.

• Offer essential after-hours support and coordinate care for hospice patients and families using customized client protocols.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license.

• Strong preference for experience in hospice, palliative, or end-of-life care.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Ability to type documentation and notes in a high-paced environment.

• Capacity to manage stress and multitask while receiving calls.

• Excellent communication and critical thinking abilities.

• Capability to work independently in a remote setting.

• Reliable attendance, proactive communication, and dependable internet connection during all scheduled shifts.

• Availability for scheduled shifts: 3:30 PM to 11 PM CST, plus rotating weekend shifts from 9:30 AM to 4 PM CST.
