Hospice Triage RN, PT, 3:30p-11p, rotating Sat & Sun 9:30a-4p CST

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions to determine suitable next steps.

• Collaborate with on-call teams to ensure coordinated care and resource allocation.

• Document all communications and interventions with precision.

• Maintain a calm and professional attitude while managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and their families.

• Coordinate timely interventions and care for patients receiving hospice services.

⛳️ Requirements

• Possession of an active multistate Registered Nurse (RN) license.

• Strong preference for experience in hospice, palliative, or end-of-life care.

• Proficiency with technology and electronic medical records (EMR).

• Ability to quickly document and take notes in a high-paced environment.

• Capability to manage stress and multitask effectively while on calls.

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Consistent attendance and proactive communication during scheduled shifts.

• Reliable internet connection during all scheduled work periods.
