Hospice Triage RN, Full-Time, Monday-Friday, 6:30am-4pm

This is a fully remote position, open to applicants in Texas.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify appropriate subsequent actions.

• Work collaboratively with on-call teams to ensure care and resource coordination.

• Thoroughly document all communications and interventions.

• Maintain a composed and professional attitude when managing urgent calls.

⛳️ Requirements

• Possess an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Must be proficient with technology and the electronic medical records (EMR) used for call documentation.

• Capable and comfortable with typing documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Strong communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• This role necessitates consistent attendance, proactive communication, and dependable internet connectivity during all scheduled shifts.
