Home Health Intake Information Coordinator

This is a fully remote position, open to applicants in California.

📋 Description

• Oversees daily operations for patient referrals and intake, including the execution of intake policies.

• Takes charge of evaluating initial patient requirements across all disciplines.

• Coordinates visit scheduling for field personnel.

• Guarantees that patient admissions comply with state, federal, and Joint Commission referral/intake regulatory standards.

• Keeps up-to-date documentation in electronic clinical records in line with agency policies, procedures, and relevant laws and regulations.

⛳️ Requirements

• A high school diploma or a general education degree (GED); or one to three months of related experience and/or training; or an equivalent combination of education and experience.

• Prior experience in performing clerical or administrative duties within a healthcare environment or a related role is preferred.

• MUST POSSESS HOME HEALTH OR HOSPICE INTAKE EXPERIENCE.
