
Home Care Physical Therapist – 1099

• Provide care and treatment to patients within your assigned geographic region.
• Assess patients’ requirements for physical therapy in the home environment and create personalized treatment plans.
• Collaborate with the physician, patient, and family to formulate a plan of care and establish treatment objectives.
• Design, prepare, and execute tailored therapy programs aimed at restoring function and encouraging independence.
• Educate family members, PTAs, Home Health Aides, and caregivers on suitable therapy methods.
• Ensure timely and precise documentation for each visit.
• Effectively communicate with patients, families, and interdisciplinary team members, particularly regarding discharge planning.
• Provide care in accordance with agency policies and professional practice standards.
• Graduate of a Physical Therapist Program.
• Licensed Physical Therapist in New York State or eligible for licensure.
• Required Certifications: First Aid, AED, and CPR Certifications.
• Prior home health experience is preferred but not mandatory - mentorship will be provided for documentation and compliance support.
• Must be able to travel to patients’ homes within the designated geographic area.
