📋 Description

• Perform virtual wellness consultations with patients from the comfort of their homes.

• Engage in proactive healthcare by carrying out detailed health risk assessments, medication evaluations, and cognitive screenings.

• Collaborate directly with Sprinters to assess vital signs and coordinate blood draws.

• Deliver comprehensive care coordination and management, which includes preventive care measures and referrals to specialists.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in Louisiana.

• Consistently demonstrates the highest standards of professionalism, integrity, accountability, confidentiality, care, and compassion to ensure the delivery of high-quality health services.

• Willingness to thrive in an innovative environment that may require quick thinking and creative problem-solving.

• Proficient with technology and comfortable using devices such as laptops or mobile phones for documenting and utilizing HIPAA-compliant messaging applications for care coordination.

• Strong written and verbal communication abilities.

• Capable of working independently or collaboratively within a team.

• Successfully pass a national background check and maintain a valid clinical license.
