Family Practice Nurse Practitioner – Indiana Licensed

This is a fully remote position, open to applicants in United States.

📋 Description

• Review the daily caseload and set up technology and tools for patient services.

• Navigate the daily schedule utilizing modern technology.

• Collaborate with clinical in-home team members during patient visits.

• Establish welcoming connections with a diverse range of patients.

• Gather pertinent patient information and perform wellness services.

• Carry out health risk assessments, medication reviews, and cognitive screenings.

• Provide patients with educational resources regarding their health and wellness.

• Assess vital signs and organize blood draws with Sprinters.

• Execute essential tasks to meet patient-specific requirements.

• Deliver care coordination and management, which includes preventive care interventions, medication management, specialist referrals, community resources, and documentation.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in Louisiana.

• Previous experience conducting wellness visits.

• Capability to work either independently or as part of a team.

• Ability to pass a national background check and valid clinical license verification.
