📋 Description

• Conduct virtual wellness consultations directly with patients

• Review the case load and prepare necessary technology

• Navigate the schedule and collaborate with the in-home team

• Establish meaningful relationships with patients

• Participate in health risk assessments and medication evaluations

• Deliver comprehensive care coordination and management

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner

• Valid Family Nurse Practitioner license in Louisiana

• Consistently demonstrates the highest levels of professionalism

• Willingness to work in an innovative environment

• Proficient with technology

• Excellent written and verbal communication abilities

• Capability to work independently or collaboratively in a team

• Successfully pass a national background check and maintain a valid clinical license
