Family Nurse Practitioner, Florida Licensed

This is a fully remote position, open to applicants in Florida.

📋 Description

• Assess the daily caseload and set up technology for patient services.

• Manage the daily schedule utilizing advanced technology.

• Collaborate with clinical in-home team members (Sprinters) who visit patients at their residences.

• Engage with a diverse array of patients and gather pertinent information.

• Conduct virtual wellness visits and health assessments for adults and/or pediatrics.

• Execute health risk assessments, medication evaluations, and cognitive screenings.

• Offer patients informative resources about their health and wellness.

• Analyze vital signs alongside Sprinters.

• Organize blood draws and carry out essential patient care tasks.

• Deliver care coordination and management, encompassing preventive care interventions, medication supervision, specialist referrals, community resources, and documentation.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in Louisiana.

• Previous experience conducting wellness visits.

• Reliability, professionalism, and problem-solving skills.

• Dedication to outstanding customer service.

• Capability to work independently while upholding high-quality standards.

• Approachable, compassionate, and empathetic demeanor in patient care.

• Readiness to engage in last-minute problem solving and think creatively.

• Proficient with technology and comfortable using laptops or mobile devices for documentation.

• Familiarity with HIPAA-secure messaging applications for care coordination.

• Excellent written and verbal communication abilities.

• Capacity to work solo or collaboratively within a team environment.

• Ability to successfully clear a national background check and valid clinical license verification.

• Required application responses regarding Nurse Practitioner state licensure, Medicare and/or Medicaid Annual Wellness Visit experience, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification.
