This is a fully remote position, open to applicants in United States.
• Deliver comprehensive one-on-one coaching to members.
• Promote behavior modifications that enhance clinical outcomes and overall health.
• Cultivate positive relationships with members and ensure an exceptional member experience.
• Empower members to manage chronic conditions independently.
• Evaluate members’ skills, knowledge, barriers, and motivation to customize education and coaching.
• Employ a Motivational Interviewing, member-centered methodology.
• Attend mandatory meetings and complete necessary online or in-person training sessions.
• Adhere to scheduling best practices to optimize coaching utilization and fulfill member needs.
• Engage in process enhancement initiatives.
• Guide less-experienced coaches by providing feedback and addressing inquiries.
• Assist in the creation of educational materials including handouts, videos, podcasts, and client-facing presentations.
• Stay updated on clinical best practices and disseminate clinical knowledge across the team and organization.
• Offer independent one-on-one diabetes education and coaching using CDCES expertise and ADCES 7 Self-Care Behaviors when working within the Diabetes Management product.
• Facilitate live group coaching Learning Events focused on diabetes when required.
• A minimum of a bachelor’s degree is required.
• Certification as a Certified Diabetes Care and Education Specialist (CDCES) must be obtained and maintained.
• Maintain 75 CEUs over a 5-year period for CDCES certification.
• Engage in ongoing professional development and continuing education.
• Implement evidence-based principles within coaching practices.
• Analyze and apply research findings effectively.
• Understand and utilize the ADCES 7 DSME/T education framework.
• Demonstrated strong knowledge of diabetes science and the latest treatment alternatives.
• At least 3 years of experience in the healthcare industry is essential.
• Deliver high-quality member education and coaching.
• Evaluate member needs, barriers, skills, knowledge, and motivation.
• Utilize a Motivational Interviewing, member-centered coaching approach.
• An advanced degree in a health-related field is preferred.
• Bilingual proficiency in Spanish and English is advantageous.
• NBC-HWC certification is a plus.
• Must be legally authorized to work in the United States without future visa sponsorship.
