This is a fully remote position, open to applicants in Arizona, +3 more states.
• Provide compassionate clinical care through real-time and asynchronous chat, along with other responsive care channels.
• Handle multiple patient interactions while ensuring accurate documentation and prompt follow-up.
• Direct and supervise patients’ medical management using a comprehensive approach, including the 5M framework.
• Collaborate effectively with primary care providers, specialists, pharmacies, and specialty care teams.
• Coordinate care with patients’ family members and caregivers as necessary.
• Evaluate and offer clinical guidance and follow-up regarding symptoms, medication inquiries, lab results, and new health issues.
• Refer patients to telephone, video, in-person, urgent, or emergency care when chat interactions are inadequate.
• Take ownership of patient needs until resolution or documented transition occurs.
• Ensure accurate and timely EHR documentation in adherence to legal, ethical, and telehealth standards.
• Recognize opportunities to enhance patient experience and outcomes through digital workflows and proactive communication.
• Participate in the development of digital workflows, standardized responses, escalation protocols, and quality benchmarks.
• Assist broader Devoted Medical service lines as required.
• Provide flexible clinical coverage across Responsive Care queues and on-call services.
• Comply with relevant laws, regulations, patient privacy standards, and telehealth guidelines.
• Advanced Practice Provider (NP or PA) with a minimum of 3 years’ experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care.
• Capability to assess AI-generated responses for clinical accuracy and safety.
• Nurse Practitioners: must possess active and in-good-standing RN and APRN licenses.
• Physician Assistants: must hold an active and in-good-standing Physician Assistant license.
• Required licensure and certification must be maintained in good standing.
• Ability to secure licenses in requested states within 90 days of hiring, starting from the signing of the offer letter.
• Willingness to commit to four ten-hour shifts weekly.
• Availability for working hours from 8am-6pm MST/PST, with flexibility for EST/CST hours.
• Previous experience in virtual care or telehealth settings is preferred.
• Experience in real-time, queue-based, or high-volume telehealth clinical environments.
• Proven ability to collaborate with interdisciplinary care teams and coordinate remote care.
• Comfort in managing complex or at-risk patient populations in a digital-first environment.
• Adaptability and curiosity in a fast-paced, dynamically changing setting.
• Must reside within one of the contiguous 50 states.
