Diabetes Educator – Certified Diabetes Care and Education Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate members for eligibility and suitability for the Diabetes Clinic.

• Conduct comprehensive assessments and follow up with members to provide ongoing education on diabetes management.

• Educate members on disease processes, medications, glucose monitoring, lifestyle counseling, and contingency planning.

• Arrange follow-up appointments with the CDCES and healthcare providers.

• Connect members and providers with relevant resources within Devoted to address clinical issues.

• Establish and work towards collaborative SMART goals with members.

• Utilize motivational interviewing techniques to foster enduring behavioral changes.

• Identify care gaps and offer appropriate resources.

• Collaborate with nurse practitioners, social workers, and pharmacists to formulate care plans and enhance medication regimens.

• Maintain accurate documentation of interactions with members and providers.

• Engage in professional development through the Diabetes Clinic education series and external continuing education opportunities.

⛳️ Requirements

• Active unrestricted Compact Registered Nurse license or unrestricted Registered Dietitian license.

• Capability to acquire additional state licenses as required.

• Current Certified Diabetes Care and Education Specialist (CDCES) certification.

• Minimum of 2 years of experience in clinically managing diabetes.

• Preferably, experience in remote diabetes management.

• Familiarity with diabetes technology, such as Continuous Glucose Monitors (CGMs) and insulin pumps.
