This is a fully remote position, open to applicants in New Jersey.
• Perform comprehensive evaluations of existing medical, behavioral, and complex social needs.
• Recognize barriers to care, such as food insecurity, unstable housing, transportation issues, and insurance constraints.
• Work collaboratively with families and multidisciplinary teams to set personalized care objectives.
• Create and uphold integrated care plans that address medical, behavioral, and social requirements.
• Ensure prompt follow-up on referrals, assessments, and treatment suggestions.
• Guide families in navigating local resources and agencies.
• Provide emotional support and counsel during difficult situations.
• Advocate for resources, eliminate barriers, and link families with available assistance.
• Establish and nurture relationships with community organizations that support children with medical and behavioral health challenges.
• Keep updated on emerging programs and resources relevant to the served population.
• Represent the organization at community events, health fairs, and resource meetings.
• Engage in scheduled interdisciplinary consultations to track patient progress.
• Carry out additional responsibilities as assigned.
• Master’s degree with a focus on social work, community/social services, and/or case management.
• 2–3+ years of relevant postgraduate experience in social service case management is required.
• At least 1 year of experience working with the pediatric population is essential.
• Familiarity with and application of motivational interviewing techniques is necessary.
• Experience offering telehealth services and engaging families in a remote setting is required.
• Comfort in working with acute populations and knowledge of school and community resources is highly desirable.
• LMSW is strongly preferred.
• Licensure/certification is preferred (LMSW, CCMC, C-SWCM).
• Bilingual Spanish is strongly preferred.
• Must reside in New Jersey.
• Availability from 8:00 am to 5:00 pm, Monday to Friday, with 2 designated days to be scheduled with the supervisor from 10:00 am to 7:00 pm.
• Willingness to travel 10% for offsite meetings, training, and educational purposes.
