This is a fully remote position, open to applicants in United States.
• Provide exceptional patient service, care coordination, and connection to community resources to ensure high-quality, patient-centered care.
• Act as a reliable liaison between diverse, underserved, and vulnerable populations and the health and social service systems that support them.
• Collaborate within an interdisciplinary care team that includes Nurse Practitioners, Nurse Care Managers, Care Coordinators, Dietitians, Pharmacists, Licensed Clinical Social Workers, Population Health Social Workers, and Psychiatrists.
• Utilize Evergreen’s integrated technology platform to assist patients in accessing services tailored to their specific needs.
• Partner with clinical teams and leadership to facilitate community outreach initiatives aimed at promoting, maintaining, and enhancing the health and well-being of patients and their families.
• Lead comprehensive care coordination focusing on identifying and addressing Social Determinants of Health while collaborating with the interdisciplinary care team.
• Aid patients in navigating the healthcare system, accessing resources and support, and achieving improved health outcomes.
• A minimum of 3 years of experience in health care coordination, community health work, or a related healthcare field.
• Credentialed as a Community Health Worker.
• Experience working within a team-based care approach.
• Strong working knowledge of community resources.
• Exceptional ability to prioritize tasks and switch between them to address the needs of patients and the overall care team.
• Self-motivated and capable of resolving issues in a professional and creative manner.
• Strong communication and teaching abilities.
• Intermediate proficiency in MS Office Suite, including Outlook and Teams.
• Ability to work effectively in a primarily remote environment:
• Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps, with Cable, Fiber, or DSL connections hardwired to the internet device recommended.
• Evergreen will provide remote employees with telephony applications and equipment necessary for their role.
