Clinical Program Manager, RN

This is a fully remote position, open to applicants in California.

📋 Description

• Act as the primary divisional contact for providers and ministry Care Coordination teams.

• Facilitate the development of safe and effective discharge plans or transitions to the appropriate next level of care for patients.

• Design and implement reporting tools to enhance information sharing between division and ministry teams.

• Cultivate collaborative relationships both within and outside of the Care Coordination teams.

• Serve as a resource and subject matter expert for both teams, offering guidance and training as required.

⛳️ Requirements

• Associate's or Bachelor's Degree in Nursing.

• Valid California Registered Nurse License upon hire.

• Minimum of 3 years of experience in Utilization Management.

• Familiarity with InterQual and MCG guidelines.
