This is a fully remote position, open to applicants in United States.
• Create, strategize, organize, and oversee training for Clinical Care Nurses, care integration team members, and primary care providers.
• Provide education on population health, value-based care, senior care, STARS quality metrics, readmission risk, chronic disease management, medication reconciliation, caregiver burnout, advance care planning, and suicide prevention.
• Integrate EMR and systems training, compliance, policy and procedure, coding and documentation, quality, utilization, customer service, team effectiveness, and communication training as required.
• Conduct training sessions in person across markets and virtually through both live and recorded formats.
• Act as faculty for clinical staff learning journeys and provider onboarding processes.
• Instruct on transitions of care, care plans, 5M geriatric best practices, and motivational interviewing.
• Evaluate medical performance via observation, call monitoring, chart analysis, and data assessment; offer coaching and performance enhancement guidance.
• Create and finalize scorecards to document the quality of Clinical Care Nurse work and share findings with stakeholders.
• Assist preceptors and the preceptor program during onboarding transitions.
• Ensure licensed RNs fulfill competency requirements and assign educational modules, shadowing experiences, and hands-on tasks.
• Work in collaboration with Regional Directors, Program Delivery, Compliance, market teams, and PCO leadership to identify training requirements.
• Represent the organization at recruitment and marketing events as necessary.
• Maintain active clinical practice in the local market for about 10% of the time.
• Interview prospective RN hires as a subject-matter expert.
• Serve as faculty for continuing education sessions or CIT Grand Rounds as required.
• Travel up to 20% for onsite training delivery, role observation, and clinical practice.
• RN license in the home state.
• Willingness to obtain licensure in additional states as necessary.
• At least five years of experience as a Registered Nurse.
• Familiarity with community health and social service agencies and resources.
• Experience as part of an interprofessional team.
• Background in training and/or development of training content.
• Proficient in Microsoft Office, EMR systems, and Zoom.
• Preferred knowledge of state Medicaid guidelines and application processes.
• Preferred experience working with the geriatric/senior population.
• Preferred experience in care management program management or implementation.
• Preferred knowledge of Salesforce and AthenaOne.
• Preferred knowledge of Team STEPPS evidence-based standards.
• Ability to reside in an existing PCO market.
• Capacity to travel up to 20%, primarily within local and regional markets.
• Minimum home internet speed of 25 Mbps download and 10 Mbps upload.
