This is a fully remote position, open to applicants in Connecticut.
• Deliver paraprofessional home health and hospice services to patients in their residences according to physician directives.
• Implement care plans established by the RN and communicate any changes in client conditions to the RN.
• Aid with personal care tasks including bathing, mouth care, denture care, skincare, grooming, toileting, transferring, ambulation, and dressing.
• Support household duties such as meal planning and preparation, feeding, light housekeeping, changing linens, bed-making, and laundry.
• Measure and document temperatures, pulse, respiration, and blood pressure as instructed.
• Conduct range-of-motion exercises and other basic procedures as an extension of assigned therapy services.
• Assist clients with the self-administration of medication when assigned by an RN and recorded in the clinical documentation.
• Foster professional relationships with clients and their families while working with minimal direct supervision.
• Maintain skills and adhere to agency policies, procedures, compliance requirements, core competencies, and the code of ethics.
• High School diploma or GED is mandatory.
• Completion of a federally compliant training program as stipulated by law/regulation.
• Successful completion of a competency evaluation program is required.
• CNA license for Connecticut is necessary.
• A minimum of one year’s experience working as an aide in an institutional, home care, or hospice environment.
• Meet agency hiring standards, including the provision of professional references.
• Capability to operate with minimal direct supervision.
• Ability to maintain a professional relationship with agency clients and their families.
• Responsibility for continually maintaining skills and following agency policies and procedures.
• Ability to demonstrate Organizational Core Competencies and adhere to the Organizational Code of Ethics and Compliance Program.
• A current driver’s license is required.
• Reliable transportation to and from the work site is essential.
