This is a fully remote position, open to applicants in United States.
• Evaluate, strategize, implement, and coordinate case management activities for members.
• Assess members' medical requirements and promote overall wellness.
• Formulate proactive strategies to enhance both short- and long-term outcomes.
• Utilize clinical tools and data analysis to assess members' needs and benefits.
• Employ clinical judgment and techniques to mitigate risk factors and obstacles.
• Conduct comprehensive assessments that consider co-morbidities and various diagnoses impacting functionality.
• Review referrals to clinical resources and interdisciplinary team members.
• Partner with supervisors and healthcare stakeholders to address challenges and achieve objectives.
• Adhere to case management protocols along with regulatory and company guidelines.
• Implement motivational interviewing techniques to optimize member engagement and evaluate health requirements.
• Advocate on behalf of the client and the member’s healthcare provider.
• Work Monday through Friday during standard business hours in the local time zone, with one to two days a week extending until 7 PM.
• Registered Nurse currently residing in the Continental United States.
• Valid nursing license with unrestricted multi-state/compact privileges and capability to be licensed in all non-compact states.
• A minimum of 3 years of clinical/nursing practice experience.
• At least 1 year of experience in medical case management, care coordination, or discharge planning.
• More than 3 years of experience with personal computers, including keyboarding, navigating multiple systems, and using MS Office Suite applications.
• Direct/hardwired internet connection to a modem/router within 7 feet of the computer.
• Minimum internet speed of 25 Mbps download and 3 Mbps upload.
• WiFi and satellite internet connections are not permitted.
• A quiet, secure, private, designated home virtual workspace compliant with CVS Health and HIPAA regulations.
• Must operate within the state and city of current residence.
• Capability to set up and utilize multiple monitors while managing several applications concurrently.
• Proficient in communicating via email, calendar invitations, Teams messaging, and virtual meetings.
• Experience with Microsoft Office 365 applications or comparable Google Workspace applications.
• Ability to access secure systems like VPN and EHR portals.
• Skill in managing strong passwords and identifying suspicious emails or links.
• Competence in troubleshooting common technical issues and reaching out to IT for assistance when required.
• Willingness to acquire new skills.
• Associate's degree in nursing is mandatory.
• Experience in telephonic case management is preferred.
• Strong communication and interpersonal skills are highly desirable.
• A BSN is preferred.
