Case Management Supervisor

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate and address the needs of patients and their families promptly.

• Build and maintain professional and collaborative relationships with patients, healthcare providers, and insurance companies.

• Encourage patient involvement in their own healthcare journey.

• Oversee high-risk patient groups and manage chronic health conditions.

⛳️ Requirements

• Valid Nursing License in the state of practice or a Compact License.

• RN is preferred; however, LPNs with Managed Care experience outside of a clinical environment may be considered.

• At least two years of clinical experience.

• Minimum of one year in care (case) management or a commitment to pursue and maintain a Care (Case) Management Certification.

• Ongoing education in Care (Case) Management and other relevant professional licensure.

• CPR/BLS certification is required.
