This is a fully remote position, open to applicants in North Carolina.
• Deliver in-reach and transition care management to individuals in institutional, congregate, or facility-based environments, such as hospitals and inpatient units.
• Inform members, families, and guardians about integrated community-based care and the services available to them.
• Facilitate access to physical, behavioral, social, and community resources.
• Provide complex care management in accordance with NCQA and necessary accreditation standards.
• Create comprehensive care plans using nationally recognized evidence-based and evidence-informed standards.
• Offer guidance and oversight to providers concerning social determinants, medical, and other member needs.
• Identify, document, and work to mitigate barriers to relocation to integrated community environments.
• Involve members in State Psychiatric Hospitals and ACHs regarding permanent supportive housing and community resources.
• Complete necessary documentation, paperwork, and software platform tasks within specified timeframes.
• Collaborate with an interdisciplinary team to address Olmstead Settlement and Tailored Plan functions.
• Evaluate and monitor member health and safety, especially during emergencies or adverse weather conditions.
• Travel within the designated catchment area as required.
• Must possess QP status.
• Must hold a qualifying full license from the NC governing board overseeing Human Services professions, or be a fully licensed RN authorized to practice in North Carolina.
• The licensed RN pathway necessitates four years of full-time experience with the MH/DD/SA population.
• A master’s degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience.
• A bachelor’s degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience.
• A bachelor’s degree in a different field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience.
• Must possess a valid driver’s license.
• Must live within 75 miles of Trillium’s Southern Region, specifically in Lenoir County.
• Must be capable of traveling within the catchment area as necessary.
• Must pass a drug test as a condition of employment.
• Resume must detail employer names, dates of service, average weekly hours, key responsibilities, educational background, and licensure/certification information if applicable.
