This is a fully remote position, open to applicants in Missouri.
• Develop, evaluate, and manage care coordination activities tailored to member requirements.
• Create or assist in the formulation of individualized care/service plans.
• Inform members and their families/caregivers about available services, benefit options, care alternatives, referrals, healthcare entitlements, and provider guidelines.
• Assess member needs, obstacles to care, and the resources that are accessible.
• Identify necessary providers, specialists, and community resources for effective care.
• Offer psychosocial and resource assistance related to employment, education, housing, nutrition, participant direction, independent living, justice, and foster care services.
• Facilitate communication between members/families/caregivers and care provider teams to guarantee prompt access to necessary care and services.
• Track progress towards care plan objectives and monitor changes in member condition or status.
• Collaborate with healthcare professionals on modifications to care plans and refer members for additional care management assessment.
• Gather, document, and uphold member data and care management activities to ensure regulatory compliance.
• Conduct on-site visits to evaluate member needs and work with providers or resources as needed.
• Perform additional responsibilities as assigned while adhering to all policies and standards.
• Bachelor’s degree and 2–4 years of relevant experience.
• Graduation from an Accredited School of Nursing if holding clinical licensure.
• Equivalent experience may be considered based on relevant knowledge, responsibilities, scope, and skills aligned with the position level.
• Current state clinical license is preferred.
• Missouri LPN / LVN license is highly preferred.
• Availability to work Monday–Friday, 8am–5pm Central.
• Capability to develop, evaluate, and coordinate care plans and services.
• Familiarity with healthcare benefits, referrals, providers, specialists, and community resources.
• Proficiency in collecting and maintaining documentation in line with state, federal, and third-party payer regulations.
