This is a fully remote position, open to applicants in Michigan.
• Oversee physiological indicators such as blood pressure, weight, and blood glucose with clinical precision.
• Achieve team objectives and standards as defined by specific metrics.
• Make a meaningful impact on long-term patient engagement in RPM programs.
• Offer education and coaching in preventive health and disease management.
• Conduct monthly wellness calls with designated patients.
• Facilitate collaborative wellness discussions with patients to establish health goals set by their Care Team.
• Administer patient messaging and alerts effectively.
• Guide patients to their treating physician for routine inquiries.
• Meet the goals of the patient engagement program.
• Adhere to appropriate escalation procedures for any urgent care requirements.
• Unrestricted RN/LPN/LVN license valid in a compact state and Michigan.
• Availability to work in the EST time zone from 8:00 AM to 5:00 PM.
• Proficiency in Spanish is advantageous.
• 1-3 years of clinical experience, patient management, or disease management preferred.
• Familiarity with various provider practices and operational workflows.
• Quick learners are encouraged to apply.
• Capability to work autonomously with minimal supervision.
• Experience with Medicare patients is beneficial.
• Background in conducting virtual visits and telephonic care management with patients.
• Interest in leadership development and growth opportunities within a dynamic organization.
• Desire to engage in an innovative clinical program and play a vital role in shaping processes and best practices for RPM patient care.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is desirable.
