This is a fully remote position, open to applicants in California.
• Act as a reliable and consistent point of contact for TAY and kinship families via phone, text, email, and digital platforms.
• Establish and maintain relationships with community partners across California for seamless referrals and collaborative problem-solving.
• Utilize micro-cash grants as tools for engagement and stabilization, while capturing structured data regarding family needs.
• Activate crisis support protocols and coordinate timely, stabilizing care in collaboration with ConnectMe and relevant partners.
• Perform comprehensive needs assessments informed by Motivational Interviewing, utilizing ConnectMe’s smart intake tools.
• Recognize change talk and adjust navigation strategies according to individuals’ readiness.
• Identify resource gaps and systemic barriers to inform strategic planning, advocacy, and resource development.
• Gather and document user feedback and satisfaction data.
• Manage an active caseload throughout various stages of the care journey.
• Collaboratively develop and track individualized action plans using ConnectMe.
• Assist individuals in transitioning to community partners, service providers, and government programs.
• Aid individuals in navigating complex systems, applications, eligibility requirements, and bureaucratic processes.
• Match individuals with culturally responsive services through ConnectMe’s technology-assisted directory.
• Support individuals facing barriers related to digital access or literacy and identify accessibility gaps.
• Implement and enhance ConnectMe tools, including technology-assisted intake, automated follow-ups, chatbots, and digital referral workflows.
• Document frontline insights to improve both the platform and practice model.
• Contribute lived experiences and human-centered design perspectives to product and innovation initiatives.
• Define technology-enabled practices and document lived-experience data through case notes.
• Accurately enter data in a timely manner while upholding privacy and ethical standards.
• Engage in team debriefs, learning sessions, case consultations, and fulfill organizational deliverables.
• Collaborate with the Innovation and Tech teams on systemic barriers, policy insights, and advocacy frameworks.
• A minimum of 3 years of experience in accessing and navigating child welfare systems and/or other social service systems.
• Ideally, 2 or more years of professional experience in direct service, case management, or navigation.
• Trained or experienced in Motivational Interviewing.
• Ability to practice Unconditional Positive Regard, cultural humility, and sensitivity to lived experiences.
• Experience navigating complex and fragmented systems.
• Personal or professional experience with child welfare systems or other social impact work, or a strong commitment to learning about these systems.
• Dedication to centering the voices and experiences of individuals with lived experience.
• Solutions-oriented mindset with the ability to adapt in a rapidly evolving organization.
• Proficient in data entry and documentation in case management, CRM, or tracking systems.
• Willingness to learn and adapt to digital tools.
• Familiarity with California’s child welfare system, county structures, or specific state programs.
• A Bachelor’s Degree in social work, counseling, human services, or a related field is required.
• Relevant lived experience and demonstrated professional competency may be considered in lieu of or in addition to formal education.
• Completion of Motivational Interviewing training is expected within the first 90 days of employment.
• Preferred: Bilingual in Spanish, both written and verbal.
