This is a fully remote position, open to applicants in Texas.
• Foster Empathetic and Consistent Engagement: Act as the main point of contact, cultivating robust and trustworthy relationships with elderly patients through regular, compassionate outbound calls.
• Clinical Data Assessment and Triage: Leverage your medical/social expertise to daily monitor and evaluate Remote Patient Monitoring (RPM) data, proactively recognizing significant trends, early warning signals, and potential clinical issues.
• Patient Education and Assistance: Skillfully inform and guide geriatric patients on utilizing RPM devices, ensuring effective communication and comfortable integration of new technology.
• Coordinate Clinical Response: Meticulously document all patient interactions and health updates in the EHR/CCM platform, and promptly escalate urgent clinical issues to the relevant licensed medical professional or care team member.
• Chronic Care Management (CCM) Supervision: Actively aid patients in comprehending, adhering to, and navigating their personalized chronic care plans and community resources.
• Comprehensive Patient Focus: Tackle patient barriers related to social determinants of health (e.g., transportation, medication adherence) that disproportionately impact our aging and remote patient demographic.
• Strong Medical Background PREFERRED: A current or past clinical certification or license (e.g., Certified Medical Assistant (CMA), EMT/Paramedic, Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), or equivalent verified healthcare experience)
• Geriatric Care Expertise: Demonstrated experience (1+ years strongly preferred) in effectively working with and caring for the geriatric population
• Exceptional Emotional Intelligence: Proven ability to exhibit deep empathy, patience, and clear, therapeutic communication, particularly with complex, chronic care patients.
• Technological Proficiency: Comfort and competence in utilizing remote monitoring tools, electronic health records (EHRs), and mobile platforms
• Independent Work Ethic: Strong attention to detail and capability to excel and manage a high volume of patient interactions independently in a dynamic, remote environment
