Care Manager, RN

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Responsible for developing, assessing, and facilitating intricate care management activities primarily focused on members with physical needs.

• Assesses member needs and identifies barriers to obtaining appropriate care.

• Creates ongoing care and service plans while collaborating with healthcare providers.

• Recognizes issues or obstacles to care and implements suitable care management interventions.

• Conducts continuous follow-up and monitoring of the member's status.

• Offers resource support to members and care managers regarding local resources.

• Facilitates care management and collaborates with relevant providers or specialists.

• Collects, documents, and preserves all member-related information and care management activities.

• Provides and/or facilitates educational resources for members and their families or caregivers.

⛳️ Requirements

• A degree from an accredited School of Nursing or a Bachelor's degree in Nursing is required.

• 2 to 4 years of relevant experience is necessary.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Knowledge of physical health is essential, and behavioral health (BH) knowledge is an advantage.
