This is a fully remote position, open to applicants in United States.
• Assist in care management activities along with designated member teams
• Conduct member outreach through phone calls and home visits
• Record care and service plans, activities, care requirements, and member documentation
• Engage with members to review care plans, outline next steps, provide resources, address inquiries, concerns, and ongoing educational needs
• Coordinate care activities with healthcare professionals, community providers, partners, members, and their caregivers
• Address inquiries, requests, and concerns from members and providers
• Collaborate with care managers, practitioners, and others to ensure service facilitation and continuity of care
• Support service assessments and screenings
• Adhere to contractual, regulatory, and state mandates as well as standards of practice
• Identify member needs and make referrals to Care Managers, community organizations, and Disease Managers
• Provide information on available benefits and resources
• Execute other assigned responsibilities
• Travel around 30%
• High School diploma or GED is mandatory
• 1–2 years of relevant experience is necessary
• For Arkansas Total Care: A Bachelor's degree in a social science or health-related field is required, or a high school diploma with a minimum of one year of experience in coordinating care for clients with developmental or intellectual disabilities or behavioral health issues
• Must live in Arkansas or in a bordering city
• A valid driver's license is essential
• A child and adult maltreatment check is required prior to hiring and regularly thereafter
• A drug screening is required at the time of hire and periodically thereafter
• Strong background in working with the Developmental Disabilities (DD) population is preferred
• Must reside in or be capable of supporting members throughout Washington, Benton, and Madison counties in Arkansas
• Willingness to travel approximately 30%
