This is a fully remote position, open to applicants in United States.
β’ Assist with care management tasks and collaborate with designated member care teams.
β’ Engage in member outreach via phone calls and home visits.
β’ Review care plans and service plans, discussing next steps, available resources, questions, concerns, and ongoing education.
β’ Coordinate care activities with healthcare and community providers, partners, members, and caregivers.
β’ Address inquiries, requests, and concerns from members and providers pertaining to care plans and service plans.
β’ Communicate with care managers, practitioners, and others to enhance member services and ensure continuity of care.
β’ Assist with service assessments and screenings while documenting member care requirements.
β’ Maintain member records in compliance with state and regulatory standards.
β’ Recognize needs and refer to Care Managers, community-based organizations, and Disease Managers.
β’ Educate members about accessible benefits and resources.
β’ Perform additional assigned tasks and adhere to relevant policies and standards.
β’ A High School diploma or GED is required.
β’ 1β2 years of relevant experience is required.
β’ For Arkansas Total Care: A Bachelor's degree in a social science or health-related field, or a high school diploma with a minimum of one year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health needs.
β’ Candidates must reside in Arkansas or a bordering city.
β’ A valid driver's license is required.
β’ A child and adult maltreatment check is mandatory prior to hire and must be repeated periodically.
β’ A drug screening is required upon hiring and periodically thereafter.
β’ Willingness to travel 30% of the time is necessary.
β’ Strong experience working with the Developmental Disabilities population is preferred.
β’ Must either reside in or be capable of supporting members across St. Francis, Cross, and Crittenden counties in Arkansas.
