This is a fully remote position, open to applicants in United States.
• Assist in care management activities and collaborate with assigned member care teams.
• Conduct member outreach via telephone and home visits.
• Discuss care plans/service plans, outline next steps, provide resources, and address questions and concerns.
• Offer ongoing education to members throughout their care/service journey.
• Coordinate care activities with healthcare providers, community partners, members, and caregivers.
• Address member and provider inquiries, requests, and concerns regarding care plans/service plans.
• Communicate effectively with care managers, practitioners, and other stakeholders to ensure member services and continuity of care.
• Assist with service assessments/screenings and document member care requirements.
• Maintain member records in compliance with state and regulatory guidelines.
• Identify member needs and refer them to Care Managers, community organizations, and Disease Managers.
• Educate members about available benefits and resources.
• Perform additional assigned tasks while adhering to company policies and standards.
• High School diploma or GED is mandatory.
• 1–2 years of relevant experience is necessary.
• For Arkansas Total Care: A Bachelor’s degree in a social science or health-related field, or a high school diploma with at least 1 year of experience in coordinating care for developmentally or intellectually disabled clients or behavioral health clients is required.
• A valid driver’s license is essential.
• A child and adult maltreatment check must be completed prior to hiring and on a recurring basis.
• A drug screening is required upon hiring and on a recurring basis.
• Must reside in Arkansas or in a bordering city.
• Ability to support members across Lincoln, Cleveland, Drew, and Bradley counties in Arkansas is required.
• 30% travel is necessary.
• Preference for candidates with extensive experience working with the Developmental Disabilities population.
• Capability to coordinate care and make appropriate referrals is expected.
• Proficiency in documenting member records and care needs in accordance with state and regulatory standards is required.
