This is a fully remote position, open to applicants in United States.
• Assist with care management tasks and collaborate with designated member care teams to guarantee provider services, continuity of care, and member satisfaction.
• Perform member outreach through phone calls and home visits.
• Record care/service plans, activities, member needs, and documentation.
• Engage with members about their care plans, upcoming steps, available resources, inquiries, concerns, and ongoing education.
• Coordinate care initiatives with healthcare providers, community partners, members, and caregivers.
• Address inquiries, requests, and concerns from members and providers regarding care plans.
• Communicate with care managers, healthcare practitioners, and others to facilitate member services and ensure continuity of care.
• Assist with service assessments and screenings.
• Refer members to Care Managers, community organizations, and Disease Managers.
• Educate members about available benefits and resources.
• Carry out additional duties as assigned and adhere to policies and standards.
• High School diploma or GED.
• 1–2 years of relevant experience.
• For the Arkansas Total Care plan: Bachelor's degree in social science/health-related field, or a high school diploma with at least one year of experience in coordinating care for clients with developmental or intellectual disabilities or behavioral health issues.
• Must live in Arkansas or a bordering city.
• Must be able to support members throughout Washington and Benton counties in Arkansas.
• Strong experience with the Developmental Disabilities population is preferred.
• A valid driver's license is required.
• A child and adult maltreatment check is required prior to hire and must be renewed periodically.
• A drug screening is mandatory at hire and must be repeated periodically.
• Willingness to travel up to 30% of the time.
