Care Coordinator II

This is a fully remote position, open to applicants in United States.

📋 Description

• Assist with care management initiatives and designated member teams to ensure the delivery of provider services, continuity of care, and member satisfaction.

• Perform outreach to members via phone calls or home visits.

• Record care and service plans, activities, member needs, and relevant documentation.

• Engage with members to review care plans, discuss next steps, provide resources, address questions and concerns, and offer ongoing education.

• Coordinate care activities with healthcare providers, community partners, members, caregivers, care managers, and practitioners.

• Address inquiries, requests, and concerns from members and providers related to care plans and services.

• Assist with service assessments and screenings, documenting members' care needs.

• Safeguard and distribute member records in compliance with state and regulatory guidelines.

• Recognize needs and refer to Care Managers, community organizations, and Disease Managers as appropriate.

• Educate members about available benefits and resources.

• Carry out additional assigned responsibilities while adhering to all policies and standards.

⛳️ Requirements

• High School diploma or GED is mandatory.

• 1–2 years of relevant experience is required.

• Candidate must reside in Indiana.

• Must be available to work during Eastern Time business hours.

• Ability to support a schedule from Monday to Friday, 8:00 a.m. to 5:00 p.m.

• Capability to conduct telephonic outreach and home visits.

• Proficiency in documenting member care needs and maintaining records per state and regulatory standards.

• Ability to identify needs and make suitable referrals.

• Compliance with policies, contractual obligations, regulatory guidelines, and established standards.
